Emergency Care Authorization

Any medical condition that is life-threatening or deemed inhumane to withhold treatment will be addressed by the veterinarian on duty at the owner’s expense. While in the care of Oakview Veterinary Medical Center, I authorize additional diagnostic tests and treatments to be performed on my pet if they are deemed necessary for the safety/immediate health of my pet. The Oakview Staff will always try to contact me first (and/or designated emergency contact), but I understand this medical care may be performed before I may be reached. I understand these additional urgent measures may result in additional costs, regardless of the outcome for my pet. I have read, understand, and agree to the above policy.*

I understand my pet’s boarding reservation is NOT a veterinary or technician appointment. If I want my pet/s to get any other services during their stay, including anal gland expression, vaccines, lab work, or exam/s, I must speak to a technician to arrange a treatment plan. If conditions are discovered that do not require immediate treatment for humane or life-threatening conditions as stated above, services will NOT be performed until able to reach me or designated contact to authorize care.*

We reserve the right to not provide certain services if your pet is becoming too stressed, there is a known or suspected medical contraindication, or safety concerns for the pet and staff.

I understand that the staff at Oakview Veterinary Medical Center will use reasonable precautions to assure my pet’s safety while in their care. I will not hold the staff or hospital responsible if the animal should injure itself, escape, fail to eat, become ill, or die.*
